cam ID

I got a camshaft from a friend I'm trying to get specs on it the numbers on the end are 1264 / 3 09 / 6210 I know it's a flat tappet hydraulic for small block 8 there is a number on shaft 21796 and 09 11
can anyone identify this cam? Thanks
