Where can you find a disk brake conversion for a Dana rear end?

What is your Dana in, (or out of)? As they were used by Mopar, Ford and I believe GM, the axle flanges may vary. My Dana 60 has "late big Ford" axle flanges for the larger Ford bearing.
Try Stainless Steel Brakes. They probably have a kit for you. I have a SS Brakes kit, and it is well made and was easy to assemble.
You said that you wanted a conversion, so if you are changing to disc brakes from drum brakes, you will also need to change the master cylinder also.

You can access a chart showing all of the different axle flange diagrams. Once you know that you can start your search. A lot of info on the website.
