GM Performance Crate Engine Identification?

I am told my engine is a 1100 mile GM Performance Crate 350...
I believe both to be true...
My question is...what EXACTLY have I got?

There is a metal tag attached to the block on the passenger side, below the exhaust Header...
My eyes are old and I was tired when I read it tonight but I can tell you that the Serial # is 5015483...there are no letters...

There is also another number that is 12498968...
It says something before it but I cant see it because it is behind the header...

The intake apears to be aluminum and apears to be a little taller than normal and has a raised GM with a raised checkered flag after it up front near the radiator hose neck on the drivers side...

I would LOVE to know exactly what I have...
Can anybody here help me find out ???
thanx for any help!

There is also another number that is 12498968...

This is the actual GM part Number
Go to any GMPP site and look for that number---and you will find out what you have.

That lil plate tells me (not absolutely sure) that it is indeed a performance crate engine-----My 454 HO has the same plate with SN & PART NUMBER.

Bryan

EDIT: Cannot actually read it on this resized pic, but:

part number
build date
serial number

and the number in the box = ?????? control?????

I must have something wrong...
a search I just did shows this...

12498968 Each $129.00
Molded Hood Protector - Low Profile, Indigo Blue
Deflect road debris and insects while helping to shield the hood of your Envoy from stone damage with this custom-designed Molded Hood Protector that provides precise fit by following your hood’s contour.

maybe I should recheck that number again ???
