hub cap wire wheel install question "help"

I have the 3 bolt mt adaptor that goes on the lug nuts, seems like when I go to tighten my lug nuts the holes are just a little big so I can almost tug on my adaptor and pull it thru. Am I doing it wrong, is their a special lug nut I should purchase, if so where can I buy them? I'm thinking maybe to double nut it with a washer or two, but checking with u guys first, thanks, Dana
