title doesn't match bill of sale

I know I should of taken care of this long ago, but I didn't and here I am now. Anyways, I bought my truck from someone at the Turkey run in 2003. I have fixed it up and want to title and register it now. I went to the DMV just to check and see what hoops they wanted me to jump through.
Now here is the problem. The guy that I bought the vehicle from, is not the guy who owned the title and he is not the dealer that the title had been reassigned too. (please don't point out the legalities of this, I know their is a statute against this but I messed up long ago) The DMV mentioned a court order, but I don't know how to start that, anyone know how to do that?
Anyways, the title was signed over by the dealer but all he did was fill out his dealer info. He didn't put in the purchaser information, so theoretically, I could fill out the purchaser information and it would look like I bought the truck from the dealer. But then I wouldn't have a bill of sale.
So right now, I have a bill of sale from the guy I bought the truck from and a title but they don't match.
I don't want to use a title company like broadway for this since I have a title, but I would like some suggestions as to how I can take care of this problem. I'd like to do this properly and I know state laws vary, but any help, suggestions, advice, similar problems will be appreciated.
