coming from a SHO owner, im confident in answering this. simply enough, you need a new clutch set. the fingertips on the pressure plate have prolly snapped and worn the face of the TOB and now the fingers are hanging onto the TOB.
hopefully the original owner took the SHO in for the clutch uograde. 89 and 90 came with a 9.25" clutch, the TSB upgraded it to a 9.75" clutch. this can easily be checked without removing the transmission, but starter removal is required. a new 9.75" clutch set can be bought at for favorite auto parts store for around the price of $270. it comes with a pressure plate, clutch disc, throwout bearing, and alignment tool. if it is the smaller clutch, youll have problems finding them, though it is cheaper.
with the new clutch set, youll more than likely need to remove the quill sleeve on the trans inout shaft quill. while some have had issues removing it, i have not. just pull back on the tangs, and twist and pull,
as for the clutch adjustment, the quadrant is notrious for not holding its position when adjusted. i had to get under the dash and engage mine using a screwdriver.

When you do get it in gear, how far off the floor do you have to let the pedal come before the clutch engages (car wants to move)?
Does this distance ever change say with the engine/tranny hot or cold?
Does the amount of freeplay travel in the pedal ever change?
Does this noise occur when lightly pressing on the pedal, or when the pedal is at full travel (clutch disengaged), or both?
It sounds like the clutch isn't fully disengaging which is why it's hard to get into gear. Most noticeable in reverse because of the lack of synchronizers in reverse. This can be caused by most anything in the clutch assembly. If I remember right that year of Fords still had a manual adjustment in the clutch pedal. You had to pull up on the clutch pedal periodically and the pedal assembly would self adjust the freeplay. Make sure if equipped, that this mechanism isn't causing the problem by not allowing enough travel to fully disengage the clutch. Should be no more than an inch or so of freeplay in the pedal.
It sounds like clutch time.

Taurus SHO clutch issue... I know, big surprise...
My brother's '89 Taurus SHO was bought for $265. It has 106,xxx miles on it and runs great. However, the clutch is a quirky little son of a gun. When you press the clutch down all the way, it makes a noise that matches the engine speed. Not neccessarily a grinding noise, more of a slight paper-like noise.
We thought it might be the clutch cable so we replaced that for a fairly cheap $30. Grabs a little better but the next day, it just doesn't grab any gear at all. I don't know how to explain it... but I'll try. I push in the clutch, put it in 1st gear, and hear massive paper-like noise. I attempt to put it in reverse and it just grinds so I don't put it in at all.
We know its not the clutch cable but it could have been needing a change anyways. So if its not a clutch cable, is it the throw-out bearing? It so definitely seems like it. My brother and my father both think its the clutch fork or my dad thinks its just a clutch job thats in order. He had a '87 Taurus MT5 - 4cylinder w/ a 5 speed - and he had that done with a clutch job for $800 and my mom got mad at him for doing that without talking to her.
Whats your opinion?
