Rear leaf spring shock question?

I have a 49' chevy 3100 truck. I bought a complete suspension kit rear leaf, mustangII front, etc. It's a TCI kit, and the instructions completely suck. I have the front all done, but I need help with the rear. I am a little confused on the shock mounting, should be straight forward, I thought? Anyone have any photos of what it should look like? A picture would really help me. The top part of the shocks mount about a foot from the inside rail and the bottom connects to the plate on the underside plate. Anyone know how the shocks should properly connect to the plate under the Ubolts?

Really depends on what type of mount you have. I've seen ones that only attach to the rear two bolts of the pad and ones the are welded directly to the pad itself. I've been trying to find pads from an S-10 which already have the shock mounts on them.

It has a tab with a hole welded onto it. It has 6 holes total. 4 are for the Ubolts one if for the shock and there is one more that I am not sure what it's there for yet?
