Is there a decent alternative to isocyanate urethane paint? I've noticed there's now water based low VOC paints out there. But I seem to remember that the early water base automotive paints didn't hold up very well. I'd rather not have to deal with paint that requires a full hazmat suit if I can avoid it. On the other hand, I don't want to have to repaint in 2 years either.

What are you trying to do? If painting a car is the task get with an automotive paint supplier and talk to him/her about the project. There are as many options out there as there are colors. I have found most stores are more then happy to lead you through the process and options. They will also advise you of the + and - of various products along with safety requirements.
