What kind of RPM’s do you visit. The combination of a bent valve stem and popped out rocker speaks to a collision between piston and valve.
A push rod coming out of the rocker for fitment reasons would simply let the valve snap seated, there would be no moment involved that would bend the stem.
This could also result from the valve sticking. Exhausts run mighty hot which eats the clearance space, cooks what little lube they get into a sticky gum, so this adds a possibility that even with revs not high enough to where the valve spring lost control the valve could be sticking open and got hit.
I greatly recommend pulling the head for both investigation of contact with the piston but, also, to correct any damage done to the guide.
